Before you buy clomid, the question that determines whether it will work at all: are you ovulating? Clomiphene induces ovulation in women who are not ovulating regularly. In a woman who already ovulates every month, it adds nothing — and in a couple whose difficulty conceiving is a sperm problem or a blocked tube, it addresses the wrong person entirely. Roughly a third of infertility is male-factor. That is why the first step is investigation rather than a prescription, and why clomid cost is a secondary consideration to whether the drug is aimed at your actual obstacle.

How it works, briefly

Clomiphene blocks oestrogen receptors in the hypothalamus. The brain reads the resulting low oestrogen signal as a shortage, increases GnRH output, and the pituitary responds with more FSH and LH — which is what drives a follicle to mature and release. It is an indirect mechanism, which is why it requires an intact hypothalamic-pituitary axis to work and does nothing in primary ovarian insufficiency.

The schedule

A five-day course starting on day two, three, four or five of the cycle, at 50 mg daily. Ovulation typically follows five to ten days after the last tablet. If no ovulation occurs, the dose is increased to 100 mg in the next cycle. Most pregnancies that happen on clomiphene happen within the first three to six ovulatory cycles; beyond six, continuing is generally not recommended because the yield falls and the risks accumulate.

Timing intercourse matters more than people expect. The fertile window is the few days before ovulation, not the day after, and ovulation predictor kits or a mid-luteal progesterone test are the usual ways of confirming that something actually happened.

A single course is five tablets, so a ten-tablet pack covers two cycles. Buying six cycles in advance is usually premature: the dose may change after the first cycle, and treatment should be reviewed rather than continued automatically.

The two risks that justify monitoring

Multiple pregnancy. Clomiphene raises the twin rate to around seven to nine per cent, against roughly one per cent naturally. Higher-order multiples are rarer but occur. Twin pregnancy carries materially higher risks of prematurity and complications for both mother and babies, and this is the main reason the drug is monitored rather than simply dispensed.

Ovarian hyperstimulation. Uncommon with clomiphene and usually mild, but abdominal distension, severe pelvic pain, rapid weight gain, nausea or breathlessness during or after a cycle need assessment the same day rather than observation.

Ultrasound monitoring of follicle number is what manages the first and catches the second. A prescription issued without any plan for it is not a complete treatment.

What it feels like

Hot flushes are the commonest complaint, affecting a substantial minority and settling after the course. Mood swings, breast tenderness, bloating, nausea and headache occur. Visual disturbance — blurring, spots, light trails — is uncommon but important: it is a reason to stop the drug and contact the prescriber rather than push through, and clomiphene should not be restarted after it without advice.

One effect is specific and often missed: clomiphene's anti-oestrogenic action can thin the endometrium and thicken cervical mucus, which works against conception even when ovulation is successfully induced. That paradox is why a woman who ovulates on clomiphene for several cycles without conceiving is usually moved on to something else rather than given more.

When it is not the right drug

Pregnancy, liver disease, undiagnosed abnormal uterine bleeding, ovarian cysts other than polycystic ovaries, and hormone-dependent tumours are contraindications. It will not work in primary ovarian insufficiency, where the ovary rather than the signal is the problem.

And it should not be the first step when the cause of anovulation has not been established. Thyroid dysfunction and raised prolactin both cause anovulation, both are easily detected on a blood test, and both are treated by correcting the underlying problem rather than by overriding it. Starting clomiphene without those results is how a treatable condition goes unnoticed for a year.

The part that is not pharmacological

Fertility treatment puts sex on a schedule, and scheduled sex under scrutiny is where desire tends to go. Couples commonly describe exactly that — intercourse becoming an obligation timed to a kit, loss of spontaneity, and in a proportion of men, erectile difficulty that appears for the first time precisely when performance is required on a particular day. What should be checked before the first cycle

Clomiphene overrides a signal. Before overriding it, the sensible question is why the signal is absent, and that question has answers that are easy to find and important to treat in their own right.

Thyroid function, because hypothyroidism causes anovulation and is corrected with levothyroxine rather than clomiphene. Prolactin, because a raised level has its own causes including pituitary adenoma and its own treatment. Day three FSH and LH, which distinguish hypothalamic causes from ovarian insufficiency — clomiphene does nothing in the latter. Androgens and glucose tolerance where polycystic ovary syndrome is suspected, since insulin resistance is treatable and improves ovulation on its own.

And the partner. Semen analysis is cheap, non-invasive and identifies a male factor in roughly a third of couples. Starting ovulation induction without it is how a year disappears.

What monitoring during a cycle involves

Ultrasound follicle tracking from around day ten, which counts how many follicles are developing — the information that manages multiple pregnancy risk. A mid-luteal progesterone around day twenty-one, which confirms whether ovulation actually occurred. Both are straightforward and both are what distinguish treatment from guesswork.

Without them, a woman can complete six cycles without knowing whether she ovulated on any of them, which is six months spent on a drug that may have been doing nothing.

The part of fertility treatment nobody prepares for

Scheduled sex changes sex. Couples describe the same sequence repeatedly: intercourse becomes an appointment timed to an ovulation kit, spontaneity disappears, and performance becomes a requirement on a particular day. A meaningful proportion of men develop erectile difficulty for the first time in exactly that context, which is then experienced as a failure at the worst possible moment.

If nothing happens over several cycles

Two different situations hide behind that. If ovulation is not occurring despite clomiphene — confirmed on a mid-luteal progesterone — the drug has failed and the next step is a different agent such as letrozole, or gonadotrophins, under specialist care.

If ovulation is occurring and conception is not, the obstacle is elsewhere: tubal, male factor, or unexplained. Further clomiphene will not find it, and continuing beyond six ovulatory cycles is not recommended for that reason.
