Propranolol for anxiety: what a beta blocker can and cannot do
- propranolol
- Film-coated tablet
- 10mg, 20mg, 40mg, 80mg
- Prescription only
Propranolol for anxiety treats the body and leaves the mind alone. A 10 to 40 mg dose taken an hour before whatever is being dreaded blocks the beta-adrenergic receptors that produce the racing heart, the shaking hands, the sweating and the voice that will not stay level. The fear itself is untouched. For performance anxiety that is exactly the right division of labour, because the physical symptoms are usually what the person is most afraid of, and the loop they feed — notice the pounding heart, conclude something is wrong, produce more adrenaline — is broken at the body end.
Where a beta blocker earns its place
Situational anxiety with prominent physical symptoms: public speaking, an examination, a performance, a first sexual encounter after a failure. It is taken once, when needed, 30 to 60 minutes ahead, and it does not need to be taken on any other day. There is no dependence, no cognitive dulling and no sedation at these doses, which is why musicians and surgeons use it and why benzodiazepines are a worse answer for the same situations.
Sexual performance anxiety is a specific case worth spelling out. A man who has lost an erection once and now approaches sex watching himself for signs of failure is generating a sympathetic response that physically opposes erection — adrenaline constricts exactly the vessels that need to dilate. Reducing that response helps. What it cannot do is remove the self-monitoring, and a man who takes propranolol and still spends the encounter watching himself will get a calm body and the same outcome.
Where it does nothing
Generalised anxiety disorder, where the problem is persistent worry rather than acute physical arousal: the evidence does not support beta blockers, and buspirone or an SSRI such as escitalopram is the appropriate treatment. Panic disorder, where it fails to prevent attacks. Depression, where it has no role and may worsen mood.
It also does nothing for the cognitive component of any anxiety. That is not a criticism of the drug; it is the reason it works best as one half of an approach, with the other half being the work described on the approccio centrato sulla persona page. At this studio the first consultation costs nothing, which is the cheapest way to find out which half of the problem you actually have.
Doses
For situational use: 10 mg to start, taken an hour before, to find out how your blood pressure responds. 20 to 40 mg is the usual effective single dose. It can be repeated on separate occasions but not stacked within a day.
For regular use in other indications — hypertension, migraine prevention, essential tremor, thyrotoxicosis — dosing is 40 mg two or three times daily and upwards, and that is a different treatment with different monitoring. A dose taken daily for months should not be stopped abruptly: abrupt withdrawal of a beta blocker causes rebound tachycardia and, in anyone with coronary disease, can provoke angina or worse.
Prices by strength and pack
Indicative prices in US dollars. For occasional use the smallest pack lasts a long time; the larger packs are for the daily indications.

Who must not take it
Asthma is the absolute one. Propranolol is non-selective, blocks beta-2 receptors in the airways and can provoke severe bronchospasm; a history of asthma rules it out even if the asthma has been quiet for years. Chronic obstructive pulmonary disease is a strong caution for the same reason.
Also excluded: significant bradycardia, second or third degree heart block, untreated heart failure, severe peripheral arterial disease, and a systolic blood pressure already low. In diabetes treated with insulin it masks the warning signs of hypoglycaemia, which turns a manageable problem into an unannounced one. Verapamil and diltiazem combined with a beta blocker can stop the heart's conduction, and that pairing needs a cardiologist.
The interaction nobody expects
Beta blockers are themselves a recognised cause of erectile dysfunction when taken daily — one of the better-documented drug causes, alongside thiazides. The effect is dose-related and does not apply meaningfully to a single 20 mg dose before an event, but it is a real consideration for anyone on 80 mg twice daily for blood pressure who has noticed a change.
If that describes you, the answer is a conversation about switching antihypertensive class rather than adding a second drug on top. Where an erection problem is already being treated, note that both a beta blocker and a PDE5 inhibitor lower blood pressure: combining sildenafil or tadalafil with propranolol is routine and generally safe, but the first attempt deserves a cautious dose rather than a confident one. The rest of this section is listed under anti anxiety.
Questions people actually ask
Will it make me calm?
It will make you look and sound calm, and that often feeds back into feeling calmer. It does not act on the brain's fear circuitry the way a benzodiazepine does, and people expecting that are usually disappointed.
How long before an event should I take it?
Sixty minutes. Thirty is sometimes enough; taking it as you walk in is too late.
Can I take it every day?
For anxiety there is no reason to, and daily beta blockade brings fatigue, cold hands and the sexual side effect described above. For other indications daily dosing is normal, and then it must not be stopped abruptly.
Is it safe with alcohol?
A small amount is not dangerous, but both lower blood pressure and the combination can produce light-headedness on standing. On a first dose, leave the alcohol out so you can tell which effect is which.
The label these figures come from
Dose ranges, contraindications, the asthma warning and withdrawal cautions follow the FDA prescribing information for propranolol, and the limited role of beta blockers in anxiety disorders follows the NICE guideline on generalised anxiety disorder and panic disorder. Use for performance anxiety is off-label; this page is information, not a prescription.

Where the facts about Propranolol come from
Propranolol: the strengths, the dosing, what the film-coated tablet form changes, the contraindications and the interaction warnings set out above are the ones in the approved labelling for propranolol — the current FDA label in DailyMed — read against the European product information where this molecule is licensed in the EU. Where a figure here differs from the one in your own pack insert, the insert for the pack you hold is the document to follow.
Propranolol touches anticipatory anxiety, panic and performance, and that part of the page was read before publication by Dr. Andrea Biserni, psychologist, psychotherapist and clinical sexologist, Ordine degli Psicologi della Toscana n. 3572 sez. A (verify in the national register). The milligrams were not: he is not a prescriber, and the dosing and safety wording for propranolol stays as the regulator wrote it.
How ordering works
Packs of Propranolol run from $26.80 to $136.20. The figure covers the medicine, the pharmacist check and tracked delivery; customs charges raised at the destination are not. Dispatch and tracking are set out under shipping and delivery, what can and cannot come back under returns and refunds, and the contract itself in the terms of sale.
Prices quoted for Propranolol are indicative; the amount charged is confirmed in writing before any payment is taken. As a prescription-only medicine it is never dispatched without a valid prescription from a licensed prescriber — send it through the prescription service and a pharmacist checks it against the strength and quantity before release. Nothing here replaces advice from your own doctor or pharmacist about propranolol, and the studio does not diagnose or prescribe by e-mail.
