Escitalopram generic: the first two weeks, and the 20 mg ceiling
- escitalopram
- Film-coated tablet
- 5mg, 10mg, 20mg
- Prescription only
Escitalopram generic is the SSRI prescribers reach for first in anxiety, and the reason is narrow: at 10 mg it does the job with fewer interactions than most of the class, because it barely touches the liver enzymes that cause trouble elsewhere. The licensed uses are major depression, generalised anxiety disorder, panic disorder, social anxiety and obsessive-compulsive disorder. What the first month feels like is not what the fourth month feels like, and the gap between those two is where most people stop too early.
The first two weeks, described honestly
Nothing improves and some things get worse. Anxiety often rises for the first week to ten days — a known effect of starting an SSRI in an anxious patient, and the reason the starting dose in anxiety is sometimes 5 mg rather than 10. Nausea, headache, loose stools and disturbed sleep are common and usually gone by day fourteen.
Mood lifts between weeks two and four, often after sleep and appetite have already settled. The practical rule is to judge the drug at six weeks on an adequate dose, not at ten days on a starting one. Anyone under twenty-five should be reviewed in the first fortnight, because the small increase in suicidal thinking associated with starting an antidepressant concentrates in that window and in that age group.
Doses, and where the ceiling is
Depression: 10 mg daily, increased to 20 mg if needed after at least a week. Anxiety disorders: start at 5 mg for a week, then 10 mg, with 20 mg available. The maximum is 20 mg, and in adults over sixty-five and in significant liver impairment it is 10 mg — not a cautious preference but a dose-related QT interval effect, which is why 30 mg is not a step that exists here even though prescribers reach for it in other SSRIs.
Take it at the same time daily, morning or evening depending on whether it makes you sleepy or wakeful. Food makes no difference to absorption.
Prices by strength and pack
Indicative prices in US dollars. Thirty tablets is a month; the three- and six-month packs are where the monthly cost falls, and most people are treated for at least six months after they feel better rather than stopping on recovery.

The sexual side effects, and what to do about them
Between a third and a half of people on an SSRI report reduced desire, delayed orgasm, or orgasm that will not arrive at all. Men also report reduced ejaculatory volume and occasionally difficulty with erection. These are the commonest reason treatment is abandoned, and they are commonly unmentioned at the review appointment because nobody asks.
There are four real options and one bad one. Waiting works for some people, since a minority of cases settle after two or three months. Reducing the dose works where the response was achieved at 20 mg and can be held at 10. Switching molecule works — bupropion is the standard move because it does not cause sexual dysfunction at all, and mirtazapine is the other. Adding buspirone has modest evidence as an antidote and is used when the SSRI is working well for mood. A PDE5 inhibitor helps the erection and does nothing for desire or orgasm, which is useful to know before buying one. The bad option is stopping the drug abruptly and saying nothing.
The ejaculatory delay this class produces is also why the same molecules appear in the men's health range: sertraline and paroxetine are prescribed off-label for premature ejaculation precisely because of the side effect everyone else is trying to avoid.
Interactions and contraindications
MAOIs are contraindicated with a fourteen-day gap either way; linezolid and methylene blue follow the same rule. Pimozide is out. Escitalopram prolongs the QT interval in a dose-dependent way, so combining it with other QT-prolonging drugs — including fluconazole at higher doses and the macrolide antibiotics — needs checking rather than assuming. Congenital long QT syndrome rules it out.
Tramadol, triptans, lithium and St John's wort raise serotonin syndrome risk. NSAIDs, aspirin and anticoagulants raise bleeding risk. Low sodium is a specific risk in older patients and presents as confusion rather than as anything that looks like a side effect.
Stopping it
Escitalopram's half-life is around thirty hours, which puts it between fluoxetine and paroxetine for discontinuation symptoms. Taper over at least four weeks, more slowly from the lowest dose, and expect dizziness, flu-like feelings and sensory disturbance if you stop abruptly. Those symptoms are not relapse: they start within days and settle within weeks, where depression returns over months.
Medication is also not the only half of the treatment. For anxiety and depression, the combination of drug and psychological therapy outperforms either alone in most of the evidence, and the studio's own work is described on the psicoterapia page. Other options in this group are listed under antidepressants.
Questions people actually ask
Why do I feel more anxious in the first week?
It is a recognised early effect in anxiety disorders, which is why the starting dose is often halved for the first week. It passes; if it is severe, the dose comes down rather than the drug being abandoned.
Can I drink on it?
There is no dangerous interaction, but alcohol worsens both sleep and mood and adds to the sedation. In the first month it is worth keeping intake low simply to be able to tell what the drug is doing.
Is 20 mg twice as good as 10?
No. For most people the dose-response curve is nearly flat between 10 and 20 mg while side effects and the QT effect are not, which is why 10 mg is where most treatment stays.
How long will I be on it?
At least six months after full recovery for a first episode, longer for recurrent depression. Stopping at the point of feeling better is the single commonest cause of relapse.
The label these figures come from
Dose ranges, the 10 mg ceiling in older patients, QT warnings and discontinuation advice follow the FDA prescribing information for escitalopram and the NICE guideline on depression in adults. This page is information, not a prescription.

Where the facts about Escitalopram come from
Escitalopram: the strengths, the dosing, what the film-coated tablet form changes, the contraindications and the interaction warnings set out above are the ones in the approved labelling for escitalopram — the current FDA label in DailyMed — read against the European product information where this molecule is licensed in the EU. Where a figure here differs from the one in your own pack insert, the insert for the pack you hold is the document to follow.
Escitalopram touches mood, sleep and the sexual side effects that decide whether an antidepressant is kept, and that part of the page was read before publication by Dr. Andrea Biserni, psychologist, psychotherapist and clinical sexologist, Ordine degli Psicologi della Toscana n. 3572 sez. A (verify in the national register). The milligrams were not: he is not a prescriber, and the dosing and safety wording for escitalopram stays as the regulator wrote it.
How ordering works
Packs of Escitalopram run from $27.40 to $188.10. The figure covers the medicine, the pharmacist check and tracked delivery; customs charges raised at the destination are not. Dispatch and tracking are set out under shipping and delivery, what can and cannot come back under returns and refunds, and the contract itself in the terms of sale.
Prices quoted for Escitalopram are indicative; the amount charged is confirmed in writing before any payment is taken. As a prescription-only medicine it is never dispatched without a valid prescription from a licensed prescriber — send it through the prescription service and a pharmacist checks it against the strength and quantity before release. Nothing here replaces advice from your own doctor or pharmacist about escitalopram, and the studio does not diagnose or prescribe by e-mail.
