Forty medicines sit in this group and almost all of them do the same job through one of four molecules: sildenafil, tadalafil, vardenafil or avanafil. Prices run from about $24 for ten tablets of generic sildenafil to a few hundred dollars for the largest packs, and the per-tablet cost drops sharply above thirty tablets. The practical question is rarely which brand but which of the four, in what strength, and in a form you will actually take.

Sildenafil and vardenafil work in a window of four to six hours and need to be taken on a reasonably empty stomach; a heavy meal delays the onset by an hour or more. Tadalafil holds for around thirty-six hours and is largely unaffected by food, which is why it suits people who do not want to plan an evening around a tablet. Avanafil is the fastest to act — often within fifteen to thirty minutes — and the shortest-lived. None of them produce an erection on their own: all four need sexual stimulation to work at all, which is the single most common reason a first dose is reported as a failure.

The oral jellies, chewables and effervescent tablets are the same molecules in forms that dissolve faster or avoid swallowing a tablet. They are not stronger. The dapoxetine combinations are a different matter: dapoxetine addresses premature ejaculation, not erection, and the combination products treat both at once, which is useful when the two problems feed each other and wasteful when only one is present.

One contraindication overrides everything else in this group. Any PDE5 inhibitor taken with a nitrate — for angina, including the sprays and patches people forget to mention — can drop blood pressure to a level that is fatal. Alpha blockers for prostate or blood pressure need a dose interval. Azole antifungals and some HIV protease inhibitors raise blood levels enough that the starting dose should be reduced. These are the three groups a pharmacist checks before anything in this category is released.

Where the difficulty is not vascular — where erections are normal on waking but fail with a partner, or the problem started with a specific relationship — medication tends to work and then stop working once it is withdrawn. Food, alcohol and why the same tablet behaves differently

Sildenafil and vardenafil are slowed considerably by a heavy or fatty meal — the tablet still works, it simply arrives late, which is how a dose gets written off as useless. Where several attempts at the maximum dose genuinely produce nothing, that is a finding worth investigating rather than a reason to try a different brand — it points to vascular disease, low testosterone or a neurological cause.

The combination that is never made

Nitrates and PDE5 inhibitors together cause a drop in blood pressure that can be fatal, and that is the one absolute rule in this section. It covers glyceryl trinitrate spray and tablets, isosorbide mononitrate and dinitrate, and nicorandil, whether taken regularly or kept for occasional chest pain. The interval matters too: at least twenty-four hours after sildenafil or vardenafil, forty-eight after tadalafil, before any nitrate is given. Amyl nitrite poppers fall under the same prohibition and are the version most often overlooked, because they are not thought of as medicine. Anyone who may need emergency nitrate treatment should be able to say when the last dose was taken.

What is here besides the PDE5 inhibitors

Most of this section is sildenafil, tadalafil, vardenafil and avanafil in different wrappers, and for most men that is where treatment starts and ends. Four groups of products exist here for the men it does not work for.

When tablets fail on several properly timed attempts, the question is no longer which tablet to try next but whether the nerve signal is arriving at all, and alprostadil answers that in twenty minutes by supplying the signal directly. When the complaint is desire rather than erection and two morning blood tests confirm a deficiency, testosterone undecanoate is the treatment — and when those tests are normal, it is not.

When the problem is timing rather than firmness, the options split by how much notice you have. A lidocaine and prilocaine spray works in five minutes, clomipramine in four to six hours, and daily sertraline or paroxetine over three weeks with the largest effect of the three.
