Trazodone for sleep and for depression: two doses, two drugs

  • trazodone
  • Tablet
  • 50mg, 100mg, 150mg
  • Prescription only

Trazodone for sleep and trazodone for depression are effectively two different medicines sharing a molecule. At 25 to 100 mg at night it is a hypnotic, prescribed off-label, working through histamine and serotonin-2 receptor blockade. At 150 to 300 mg a day it is an antidepressant, which is what it is actually licensed as. The dose decides which drug you are taking, and most of the confusion around it comes from people reading about one dose range while taking the other.

The low-dose version

25 to 50 mg half an hour before bed, up to 100 mg. It shortens the time taken to fall asleep and reduces night waking, and unlike the benzodiazepine-type hypnotics it does not cause dependence and does not lose its effect over weeks. It also does not suppress REM sleep the way the older sedatives do.

The cost is morning grogginess, which is dose-related and the main reason people abandon it. Starting at 25 mg rather than 50 solves that in most cases. Taking it earlier in the evening rather than at the moment of lying down helps too, because the peak arrives in one to two hours.

What it does not do is treat the reason for the insomnia. Where sleep is broken by anxiety, by alcohol, by shift work or by a snoring partner, a sedative produces sleep without addressing any of it, and the sleep goes again when the tablet stops. Cognitive behavioural therapy for insomnia outperforms every drug in this category over six months and is the first-line treatment in every current guideline.

The antidepressant version

150 mg daily, built up to 300 mg if needed, usually in divided doses or as a single night-time dose. It is a reasonable choice for depression with prominent insomnia and agitation, where the sedation is a feature rather than a side effect. It causes markedly less sexual dysfunction than the SSRIs, which matters in this catalogue, though it is not entirely free of it.

At these doses the practical problems are postural hypotension and daytime sedation, both of which improve over the first fortnight and both of which argue for slow titration in anyone over sixty-five.

Prices by strength and pack

Indicative prices in US dollars. For sleep, a 30-tablet pack of the 50 mg strength is a month at one tablet a night; the higher strengths belong to the antidepressant dose range.

Priapism: the warning that belongs on this page

Trazodone is the antidepressant associated with priapism — a painful erection that will not subside — through its alpha-adrenergic blocking action. It is rare, estimated at something in the order of one in several thousand men treated, and it is a surgical emergency: tissue damage begins within hours and permanent erectile dysfunction is the consequence of waiting.

The rule is simple and absolute. An erection lasting more than four hours, or any erection that is painful and unwanted, means the emergency department the same hour, not the morning. Men starting trazodone should be told this explicitly rather than left to find it in a leaflet, and a man with sickle cell disease or another condition predisposing to priapism should be on something else. It is also the reason not to combine trazodone with an intracavernosal treatment such as alprostadil without a urologist's involvement.

Interactions

MAOIs are contraindicated with fourteen days clear. Trazodone prolongs the QT interval, which rules out combinations with other QT-prolonging drugs and makes it a poor choice in congenital long QT syndrome. It is cleared by CYP3A4, so clarithromycin, itraconazole, ketoconazole, ritonavir and fluconazole raise its levels and call for a dose reduction.

Alcohol and any other sedative add to the drowsiness in a way that matters at the wheel the following morning. Combining it with flibanserin is specifically awkward, since both lower blood pressure and both sedate. Serotonin syndrome risk applies with SSRIs, triptans and tramadol.

Where sleep, mood and sex meet

Insomnia, low mood and loss of desire arrive together often enough that treating one in isolation usually disappoints. Depression flattens libido before anything else; broken sleep lowers testosterone measurably in young men within a week; and an SSRI prescribed for the mood can take care of whatever desire the depression left. Trazodone is sometimes the useful compromise in that triangle, because it treats sleep and mood without the sexual cost of the serotonergic drugs.

It remains a drug, not a resolution. Where the sleep problem is anxiety and the anxiety has a cause, the work described on the psicoterapia page is what changes the pattern; the first consultation at this studio is free. Other options in this group are listed under antidepressants, and buspirone is the non-sedating alternative where daytime anxiety is the real complaint.

Questions people actually ask

Is it addictive?

No. There is no dependence and no tolerance to the sedative effect, which is why it is used for long-term insomnia where a benzodiazepine-type hypnotic is not.

Why do I feel drugged in the morning?

The dose is too high or taken too late. Try 25 mg, taken two hours before bed rather than at bedtime.

Can I use it occasionally?

For sleep, yes — it works from the first dose and needs no build-up. As an antidepressant it does not work that way and has to be taken daily.

Does it cause sexual side effects?

Far fewer than the SSRIs, and that is a reason it gets chosen. The exception is the priapism risk in men, which is rare but needs to be recognised immediately if it happens.

The label these figures come from

Dose ranges, the priapism warning, QT effects and CYP3A4 interactions follow the FDA prescribing information for trazodone, and the first-line status of cognitive behavioural therapy for insomnia follows the American Academy of Sleep Medicine practice guidelines. The use for sleep described here is off-label; this page is information, not a prescription.

Where the facts about Trazodone come from

Trazodone: the strengths, the dosing, what the tablet form changes, the contraindications and the interaction warnings set out above are the ones in the approved labelling for trazodone — the current FDA label in DailyMed — read against the European product information where this molecule is licensed in the EU. Where a figure here differs from the one in your own pack insert, the insert for the pack you hold is the document to follow.

Trazodone touches mood, sleep and the sexual side effects that decide whether an antidepressant is kept, and that part of the page was read before publication by Dr. Andrea Biserni, psychologist, psychotherapist and clinical sexologist, Ordine degli Psicologi della Toscana n. 3572 sez. A (verify in the national register). The milligrams were not: he is not a prescriber, and the dosing and safety wording for trazodone stays as the regulator wrote it.

Trazodone was last checked against the trazodone label on 6 October 2026. If something here no longer matches it, write to [email protected] and the page is corrected or the sentence removed — the procedure is in how we source our information. A suspected side effect from trazodone should also go to FDA MedWatch or, in Italy, to AIFA; see medicine safety. Trazodone is prescription-only and is not released without one.

How ordering works

Packs of Trazodone run from $29.20 to $165.60. The figure covers the medicine, the pharmacist check and tracked delivery; customs charges raised at the destination are not. Dispatch and tracking are set out under shipping and delivery, what can and cannot come back under returns and refunds, and the contract itself in the terms of sale.

Prices quoted for Trazodone are indicative; the amount charged is confirmed in writing before any payment is taken. As a prescription-only medicine it is never dispatched without a valid prescription from a licensed prescriber — send it through the prescription service and a pharmacist checks it against the strength and quantity before release. Nothing here replaces advice from your own doctor or pharmacist about trazodone, and the studio does not diagnose or prescribe by e-mail.
